Clinical Nurse Consultant - Voluntary Assisted Dying Care Navigator Service
Salary: $141,535 per year
Job type: Permanent Part‐time appointment – 72 hours per fortnight (0.9 FTE) as part of a job share arrangement.
Location: Fiona Stanley Hospital (FSH), Western Australia.
Key Responsibilities
* Coordinate Voluntary Assisted Dying (VAD) services for SMHS inpatients and outpatients.
* Assist patients, families and SMHS staff in VAD and end‐of‐life care choices.
* Provide initial assessment and discussion with patients/families to ascertain their understanding of the VAD process.
* Facilitate access to VAD clinicians for assessment and coordinate the VAD process when requested.
* Collaborate with treating teams to encourage completion of patient care goals, referral to palliative care and advance care planning discussions.
* Ensure safe delivery of VAD services within SMHS.
* Deliver education and training for SMHS staff to align with policies, standards and legislation regarding VAD.
* Implement changes to clinical practice and initiate research and quality improvement activities.
* Monitor, identify and report on risks associated with VAD and end‐of‐life care.
Eligibility
Applicants must have a valid working visa (for fixed‐term appointments) or be an Australian citizen, permanent resident, or hold a visa that allows indefinite work rights in Australia (for permanent appointments).
