The Role

The Mental Health Nurse will undertake the role of Clozapine Care Coordinator (CCC), providing specialist clinical support for General Practitioners (GPs) in monitoring and responding to clozapine therapy.

The role will operate from multiple GP clinics and focus on comprehensive monitoring of mental and physical health, proactive management of clinical deterioration, medication education and facilitation of access to multidisciplinary care.

The role ensures recovery oriented, evidence based mental health care in partnership with multidisciplinary teams, patients, families, carers, and all stakeholders.

The CCC ensures integration of GP led care while supporting patient centered approaches that promote meaningful engagement in life and recovery. The service will be integrated with public mental health services and health, justice, education, and welfare sectors.

Duties and Responsibilities of the PositionClinical Monitoring and Support

Coordinate care with Mental Health Providers, including (but not limited to) GPs, psychiatrists, pharmacies and pathology services to ensure safe monitoring and adherence.

Provide expert clinical support to GPs in monitoring patients on clozapine therapy, including monitoring medication and mental, physical and metabolic health.

Ensure culturally safe, trauma-informed care tailored to priority populations.

Identify early signs of clinical deterioration and intervene promptly within the scope of practice.

Support the coordination of clozapine blood tests and other required investigations, liaising with pathology services and pharmacies.

Troubleshoot clinical issues and prepare relevant information prior to medical appointments.

Care Planning and Coordination

Conduct patient intake, assessment and education for individuals prescribed clozapine therapy.

Maintain care plans, clinical records and monitoring alerts in accordance with WA clozapine guidelines.

Support patient engagement and adherence, including outreach where required.

Facilitate communication and shared-care planning between GP practices and BSH multidisciplinary teams.

Develop, implement and maintain single, standardised multi-agency care plans aligned with GP mental health treatment plans.

Conduct comprehensive assessments covering mental health, physical health and personal support to inform treatment planning.

Organise follow-up appointments, referrals and specialist consultations, including shared-care arrangements where appropriate.

Coordinate multidisciplinary care, including liaising with psychiatrists, allied health professionals and state-based services (e.g., EMHS, NMHS, SMHS, CAHS, WACHS).

Patient Education and Engagement

Aid, education and practical support to patients regarding medication management and clozapine therapy.

Assist patients in understanding treatment options and making informed decisions about their care.

Promote patient self-management and engagement with both physical and mental health interventions.

Quality and Outcomes Monitoring

Participate in service evaluation, data collection and continuous quality improvement initiatives.

Measuring standardised treatment outcomes, patient reported outcomes, GP charting etc. To review and troubleshoot prior to the client's appointment.

Maintain accurate and up-to-date clinical records in line with professional and organisational standards.

The successful candidate will have

AHPRA Registered Nurse, Clinical Nurse or Clinical Nurse Specialist.

A minimum of 3 years' experience in mental health nursing and/or post graduate mental health education.

Experience with high acuity mental health care such as patient diagnosed with psychotic disorders.

Experience in high-risk medication management including clozapine therapy.

Ability to work independently and in common settings.

Prior knowledge in working with primary care.

Experience in working collaboratively with authorised, approved and accredited mental health practitioners, including GPs, psychiatrists, clinical psychologists and/or allied health professionals.

Experience in integrating care with public mental health services and broader sectors, including justice, education and welfare, where relevant.

Competency in trauma-informed care and culturally safe practice.

Experience in engaging priority populations (Aboriginal, CaLD, LGBTIQA+, socioeconomically disadvantaged).

Requirements to Employment

AHPRA Registration.

Current National Police Clearance.

Working with Children Check.

Current unrestricted eligibility to work in Australia.

Current unrestricted driver's license and own vehicle to be used for work purposes.
