About the Role
We are seeking a dedicated Child Wellbeing Intake and Assessment Practitioner to join our team. As the first point of contact for families seeking help, you will play a vital role in assessing their needs, providing guidance, and connecting them with relevant services.
Key Responsibilities:
* Critical Response and Intervention: Conduct assessments, screenings, and crisis responses to support vulnerable children and families.
* Multidisciplinary Collaboration: Work closely with a multidisciplinary team to identify and prioritize risks, ensuring effective case management.
* Brokerage and Referral: Apply for brokerage on behalf of clients, facilitating access to necessary resources and services.
* Variety of Service Delivery: Provide services via phone, in-person, and outreach, adapting to meet the diverse needs of clients.
* Specialist Connections: Refer clients to specialist practitioners or services as needed, promoting holistic support.
