Care Coordinator (Registered Nurse)
Churches of Christ – Fair Haven Aged Care, Hervey Bay QLD (Permanent Part‐Time, Afternoon Shift)
About the Role
* Lead and coordinate comprehensive resident assessments within a Positive Wellbeing Model.
* Ensure care plans are developed, implemented and reviewed in line with assessment outcomes and organisational timeframes.
* Coordinate the accurate collection and submission of clinical and care data.
* Monitor clinical documentation to ensure it accurately reflects care delivery and meets compliance requirements.
* Coordinate care delivered by external service providers.
* Support the review and maintenance of clinical programmes, initiatives and continuous improvement activities.
* Provide leadership, workforce oversight and day‐to‐day clinical guidance within your area of responsibility.
About You
* Current AHPRA registration as a Registered Nurse.
* Minimum three years post‐graduate experience.
* Current First Aid and CPR certification.
* Current Police Check, or willingness to obtain.
* Australian work rights (sponsorship unavailable).
