We are looking for a passionate Service Delivery Manager to support the Aged and Community Care Program. The Team Leader will contribute to the overall direction and management of Aged and Community Care Services as a member of the Senior Leadership Team.
The successful candidate will ensure the delivery of service performance in line with program objectives and targets in consultation with the Manger. They will assist the team in achieving day-to-day operational service delivery. Additionally, they will provide operational support to staff in conjunction with the manger by developing, implementing, measuring and monitoring set KPI's
This position is based at Cranbourne and Pakenham but may be required to work at any of the Monash Health sites.
Key responsibilities:
* To manage and develop the Aged and Community Care Service to meet the needs of our clients;
* To lead and motivate a team of professionals to achieve high-quality service outcomes;
* To work collaboratively with other teams and stakeholders to ensure seamless service delivery;
* To identify and implement opportunities for continuous improvement and innovation;
* To monitor and report on service performance, identifying areas for improvement and implementing corrective actions as needed.
