The Access Aged Care team is currently offering co-managed primary care services to residents in Aged Care facilities throughout Australia.Your New RoleIn this role, you will provide advanced nursing assessment, diagnosis, care, and intervention to Aged Care Residents within the scope of a Nurse Practitioner. This position involves on-site work with the possibility of some telehealth responsibilities. The Nurse Practitioner delivers high-level clinical nursing, leadership, management, and governance to patients. Collaborating with the wider medical team, the Nurse Practitioner assesses, diagnoses, treats, and manages patients' health concerns, emphasizing preventative health care.
