About Moorditj Koort

Moorditj Koort is an Aboriginal community-controlled organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples across Western Australia. We deliver culturally secure primary health care, chronic disease support, health promotion, outreach, SEWB programs and community-led initiatives.

Our vision is to support healthy, strong and flourishing Aboriginal communities through a holistic, one-stop-shop model grounded in culture and community voice.

About the Role

We are seeking a passionate, community-focused Health Promotion Officer to deliver culturally strong prevention, health education, and lifestyle programs across our sites.

You will work closely with families, Elders and community members to:

* plan, deliver and evaluate health promotion activities
* support participation in healthy lifestyle programs (walking groups, yarning sessions, nutrition/exercise)
* strengthen pathways to GP Management Plans, Adult Health Checks and chronic disease programs
* increase awareness of Closing the Gap initiatives
* build trusted relationships with community
* promote MKAC programs at events and community gatherings
* gather feedback to ensure programs stay community-led
* support outreach and engagement activities
* maintain accurate records and contribute to program reporting
* champion culturally safe practice across all interactions

This is a hands‐on, community-facing role ideal for someone who is culturally grounded, confident engaging with community, and passionate about health and wellbeing.

What You'll Bring

Mandatory Qualifications

* Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice (or equivalent qualification leading to AHPRA registration)
* Valid AHPRA registration

Essential Skills & Experience

* Experience working with Aboriginal and/or Torres Strait Islander communities
* Strong communication and relationship‐building skills
* Understanding of culturally secure and person‐centred care
* Experience in health promotion, community engagement or lifestyle programs
* High standard of record‐keeping and reporting
* Proficiency with Microsoft Office and electronic medical record systems
* Ability to work independently and as part of a multidisciplinary team
* WA Driver's Licence, National Police Clearance (within 3 months), WWCC, Vulnerable People Check
* COVID‐19 vaccination (as per current directives)
