About the role
As part of our Rapid Access Chronic Disease Clinic, you'll work with adults requiring wound care, respiratory support and diabetes management across multiple care settings. You'll use reflective, evidence-based practice to assess, plan and deliver high-quality nursing care while collaborating closely with a multidisciplinary team. You'll also contribute to quality processes, education, research and clinical leadership.
Responsibilities
Provide safe, person-centred nursing care across clinic, community and home-visiting environments.
Assess, plan, implement and evaluate care in partnership with patients, families and the healthcare team.
Collaborate with multidisciplinary colleagues and make timely referrals to support holistic care.
Contribute to clinical guidelines, quality improvement, incident management and safe work practices.
Provide supervision, mentoring and support to students and junior staff.
Participate in education, health promotion and evidence-based practice to enhance care outcomes.
Qualifications
Deliver accountable, person-centred nursing care and uphold Nursing and Midwifery Board of Australia (NMBA) standards.
Draw on evidence-based practice to assess, plan and evaluate clinical care.
Contribute to safe systems by following clinical guidelines, identifying risks and supporting quality improvement.
Engage in lifelong learning and provide education to patients, carers and colleagues.
Offer supportive leadership, contributing to a positive, collaborative team culture.
Work confidently within a multidisciplinary environment to achieve best-practice outcomes.
Hold a Bachelor of Nursing (or equivalent) and current AHPRA registration.
What we offer
