Role details The Home and Community Care (Bowen) at the Bowen Hospital is seeking applications for a Assistant Home Care Coordinator to join their team on a permanent basis.

Your opportunity To be an active member of our Bowen District Home Care Service, to be supported by a highly motivated, consumer focused Home Care Coordination Team.
To liaise and interact with other like-minded Community Based Service Providers and Health Professionals.
To assist in the coordination, rostering, implementation, monitoring and reviewing of a range of flexible home-based support services to eligible clients of the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP), with the aim of enhancing the independence of clients.
This is to be in line with care plans developed for each client.
About the Team Bowen District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible and goal orientated basic support services.
Our service is also an Approved Aged Care Provider, providing Home Packages to eligible consumers under Client Directed Care.
Bowen District Home Care Service provides basic support services, predominately Domestic Assistance and Social Support to approximately 300 consumers within the Bowen / Collinsville district.
Potential CHSP consumers are registered and screened by "My Aged Care" and if eligible referred for assessment by a Regional Assessment Service (RAS).
Assessment is based on assisting frail aged people, people with a disability and their carers to continue to live in the community as independently as possible.
The Bowen District Home Care office is located in the Bowen Hospital Building, Gregory Street Bowen.
