About the Role
As the Participant & Staff Liaison, you will act as a key point of contact for participants and their support teams, ensuring supports are coordinated effectively and delivered in line with participant needs and goals.
This is a dynamic role that combines participant engagement, staff coordination, support planning, administration, and problem solving.
Key Responsibilities
* Build strong rapport with participants and act as their primary contact for support-related enquiries.
* Conduct participant intakes and assist in identifying support needs.
* Support participants with changes to supports, rostering, and funding availability.
* Undertake support planning and risk assessments with participants.
* Complete progress reports, end-of-plan reports, surveys, and governance documentation.
* Respond to participant enquiries via office phones and messages.
* Assist with incident reporting and compliance requirements, including NDIS Quality and Safeguards reporting where required.
* Maintain accurate records and ensure participant supports are appropriately claimed through the CRM system.
* Match participants with suitable support person/s.
Selection Criteria
* Cert IV in Mental Health, or other equivalent qualification.
* Strong organisational and time management skills.
* Excellent interpersonal communication skills.
* Possession of a current, valid NDIS Worker Screening Check clearance and/or national police clearance as may be required.
* A current, valid Western Australian C Class Driver's Licence.
* Possession of a current valid Working with Children Check as required.
* Possession of a current COVID-19 Vaccination Certificate as required.
* Willingness to volunteer your time toward Lamp Inc. fundraising days and events, as reasonably requested.
* Commitment to fully participate in ongoing professional development and supervision as required.
* Commitment to fully comply with Work Health & Safety legislation, policies, procedures and guidelines.
* Senior First Aid.
* Demonstrated lived experience in overcoming mental health impairments.
* Experience in supporting individuals within the community, and their own homes.
* Must reside in the Busselton region.
* Position is based on 5 days (35 hrs) per week, from 9 am to 4:30 pm.
