About the Role
The Senior Clinician Social Worker – Ambulatory Palliative Care will work closely with other palliative care health professionals, to ensure holistic and supportive care is available, including the delivery of evidence based best practice palliative care, end of life care and bereavement.
This district role will work within the ambulatory care clinics, which aim to provide patient centred interventions, support holistic psychosocial and functional review, goal setting, education, and facilitates onward referral pathways as appropriate.
Must hold a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers.
Demonstrated advanced clinical knowledge, skill and significant clinical experience in the provision of social work services in a palliative care setting, at Level 2 or higher.
Currently holds a valid NSW driver's licence (P1 and P2 licence acceptable)
