Intake and Assessment Specialist to support the Home Care Team
Act as a primary contact for clients of CHSP and other programs, supporting intake, enquiries, concerns and changes to client needs.
Respond to all clients and representatives in a responsive and professional manner.
Conduct comprehensive, person-centred assessments of client needs, suitability, and eligibility for Aged Care Services.
Triage clients based on vulnerability, urgency, and level of need to ensure appropriate prioritisation.
Identify and manage risks to clients and staff during intake and review processes.
Complete scheduled and reviews for existing clients.
Maintain accurate, timely records and reporting in line with the CHSP Funding and Program Guidelines, Aged Care Quality Standards, NDIS Legislation etc.
Promote Mercy's Healthy Aging services to support clients across their aging journey.
What you'll bring
* Excellent time management, organisational and communication skills, with the ability to adapt and problem solve.
* A good Understanding and experience of the Home Care funding programs and eligibility eg CHSP, NDIS, Private and Brokerage arrangement etc.
* A compassionate, flexible and positive approach to effective patient care and management.
* The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
You will also be required to provide evidence of, or in the process of obtaining:
* A current NDIS Check.
