**About the role**
The focus of this position is to provide counselling and emotional support to people living with mild to moderate dementia and their family carers including running carer support groups.
The aim is to facilitate the emotional, cognitive, and social adaptation to the diagnosis of dementia for both, the person living with dementia and their family carers.
The Social Worker will work closely with the team involved in the dementia specific programs to ensure the delivery of appropriate support for both the person living with dementia and their family carer.
This role will be based between our Inner West and Kogarah offices/centres.
- Provide individual emotional support/counselling for people living with mild to moderate dementia and their family carers
- Provide guidance and enable effective communication between the person with dementia and their family carers and make appropriate referrals when necessary
- Organise and chair regular meetings for all participants living with dementia and facilitate peer support
- Organise and chair fortnightly carer support groups and facilitate peer support
- Provide support and guidance that will assist in maintaining the emotional wellbeing of staff and volunteers involved in service delivery
- Support in developing individual care plans for people with dementia and their carers
- Support in developing behaviour management plan if required.
- Provide a safe environment which empowers people to respectfully share information or ask questions
- Responding in a timely manner, understand the unique parameters of the request and action
- Ensure compliance with Work Health and Safety legislation, regulations and organisational policies and procedures across the service
- Demonstrate professionalism in all situations with both internal and external stakeholders
- Assist with establishing practices that promote organisation culture and vision, recognising organisational diversity and opportunities to innovate.

**About You**
- Tertiary education in Health, Social Work or Counselling
- Demonstrated experience in Social Work or Counselling
- Demonstrated understanding of the needs of aged care clients and their families
- Strong knowledge of the New Aged Care Quality Standards
- Excellent communication and interpersonal skills
- Understanding of best practice programs and support mechanisms
- Experience building and maintaining professional, client-focused relationships with stakeholders
