**What you'll be doing**

The Chronic and Complex Care Transitional Nurse Practitioner/ Nurse Practitioner (TNP/NP) is a Registered Nurse authorised to function autonomously and collaboratively in an advanced and extended clinical role within the specialty area.
The TNP/NP provides leadership that facilitates the ongoing development of clinical practice. The TNP/NP initiates and utilises findings of research in the provision of clinical services, contributes to the development and delivery of specialty related education programs and participates in formal processes for the strategic and operational planning for clinical services.
The TNP/NP role includes focused assessment and management of patients using nursing knowledge and skills and may include, but is not limited to, the direct referral of patients to other health professionals, prescribing medications and ordering diagnostic investigations. The TNP/NP role is grounded in the nursing professional values, knowledge, theories, and practice and provides innovative and flexible healthcare delivery that complements other healthcare providers.
The Chronic and Complex Care - Heart Failure TNP/NP will provide a focused assessment and management of heart failure patients across the Shoalhaven and Milton regions. The scope of practice of the TNP/NP is determined by the context and clinical guidelines within which the TNP/NP is authorised to practice. The TNP/NP is expected to engage with a Community of Practice for Nurse Practitioners.

This role is a **Category A **role. Preferred applicants will be reviewed for compliance and required to maintain compliance with the screening, assessment and vaccination requirements, relevant to your position, as outlined in NSW Health policy directive on Occupational Assessment, Screening and Vaccination against Specified Infectious Diseases.
For positions designated as child-related employment, or positions that may result in proximity to children, you will be required to obtain a Working with Children clearance number from the Office of the Children’s Guardian.

**Selection Criteria**:
- Registered nurse with a current licence to practice and authorised as a Transitional Nurse Practitioner/ Nurse Practitioner from the Australian Health Practitioners Regulation Agency (AHPRA), and current NSW Driver’s License (if required by service).
- Tertiary qualification at a Master’s Degree Level or working towards the same. Post graduate qualifications in cardiac nursing and extensive experience in Cardiac Nursing.
- Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Demonstrated ability to implement changes and develop direction in clinical practice by the initiation, utilisation and review of evidence-based research.
- Demonstrated leadership in strategic and clinical service planning and expertise in the management of organisational and cultural change.
- Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
- Proven clinical expertise in complex client-centred consultancy with the ability to work independently
