Job Overview

Position: Supported Living Facilitator – Disability Services, NDIS, Casual
Location: Toowoomba
Remuneration: $43.23 p/hr + Super + NFP Salary Packaging

This role supports people with disabilities, focusing on personal care, choice and quality of life, while promoting their autonomy.

Responsibilities
* Work in partnership with the person being supported, their family and informal support networks to attain the hopes and aspirations they have established for their lives.
* Provide support in everyday living situations, including personal care as identified, with an emphasis on choice and enhancing quality of life.
* Participate in the development, implementation and review of individual plans that focus on the life hopes and aspirations of people.
* Enable people to attain personal autonomy over their own lives and the services they receive.
* Recognise and uphold the right to respect, privacy, dignity and confidentiality of the person and their family.
Qualifications & Requirements
* Relevant industry qualification (e.g., Certificate III in Disability or Individual Support) desirable.
* Experience in the disability, aged and community sector is ideal but not essential.
* Current NDIS Worker Screening check and Blue Card or in the process of obtaining.
* Current open driver's licence.
* Current CPR and First Aid certificate (required before you start).
* Ability to work effectively in a team‐oriented environment and provide excellent support to clients and co‐workers.
* Flexibility to work varied hours and days and reliability to meet the needs of the people we support.
* Effective communication in spoken and written English.
