Hours: 16 hours per fortnight.
The Motor Neurone Disease (MND) Share Care Worker is a regional role in the Hume Region of Victoria and is an integral part of the Palliative Care team, whose purpose is to raise the quality of life for people living with MND by:
* Promoting early referral to palliative care services
* Supporting, developing and delivering education to palliative care services providers
* Providing coordination between palliative care services, MND clients and families in the Hume region
The MND Share Care Worker also supports people living with MND, their families, health professionals and community health care providers. As MND progresses, care needs can rapidly change. Each individual will have their own unique range of symptoms and requirements for support. The MND Share Care Worker assists in navigating these pathways and works closely with the MND Victoria Advisor and Support Coordinator.
