360 Health + Community is a person-centred health service providing care and support to people when, where and how they need it most. Our teams deliver programs across Western Australia to help body and mind including mental health care for young people in our three headspace centres along the beautiful coastline south of Perth in Fremantle, Rockingham and Mandurah. We are excited to be expanding our service delivery to include the new position of Youth Wellbeing Clinician - Outreach & Intervention at headspace Rockingham.
As a Youth Wellbeing Clinician, you will be both in the centre and out in the community, supporting young people aged 12 to 25 who may be at risk or navigating difficult transitions in care. You will walk alongside young people and families, providing flexible, trauma informed, and recovery-oriented support that helps them stabilise, set goals, and connect with the right ongoing services.
Your work will include:
* Providing short term, flexible outreach and intervention to young people and families.
* Conducting comprehensive assessments and developing care plans that are youth centred and strengths based.
* Supporting families and carers to understand mental health and strengthen protective relationships.
* Building strong partnerships with schools, general practitioners, community organisations, and other mental health services to coordinate care.
* Delivering evidence informed interventions and warm referrals that are culturally safe and inclusive.
* Engaging priority groups including Aboriginal and Torres Strait Islander young people, young people from culturally and linguistically diverse communities, and those who identify as LGBTQIA+.
* Facilitating education and engagement activities that promote mental health, wellbeing, and stigma reduction.
To be successful in joining our team, you will need:
* Tertiary qualification and professional registration in Psychology, Social Work, Occupational Therapy, Counselling or other approved field
* Current Working with Children Check and National Police Certificate less than 6 months old (or be willing to apply)
* Experience working in the youth mental health sector with young people who are high-risk, LGBTQIA+ identifying people, First Nations people, and/or Culturally and Linguistically Diverse people
* Comprehensive knowledge of and ability to provide evidence based, developmentally appropriate clinical services to young people and their families and carers
* Experience in conducting thorough assessments and matching need to appropriate referral pathways.
* Excellent communication skills to build rapport with stakeholders, actively encourage the young person to participate in decisions relating to their own care, and present information at clinical meetings
