Candidates for medical specialist positions in out-of-hospital care—such as home-based care, mobile clinics, or community-based specialist outreach—require a specific blend of advanced clinical skills, high adaptability, and strong patient-centered interpersonal abilities. They must be comfortable operating independently, often without immediate access to advanced diagnostic imaging or laboratory, while delivering high-quality, safe, and compassionate care in non-traditional settings.

In the context of healthcare system changes within Western Sydney local Health District —such as shifting from acute to out of hospital care models and increasing digital integration the ideal candidate will have a mix of traditional clinical expertise and advanced non-technical systems thinking—integrating knowledge, skills, and a holistic perspective—to improve care delivery, patient outcomes, and system efficiency. The successful candidate will need to act as change agent, demonstrating leadership, adaptability, and a commitment to patient-centered, collaborative care.

As the Medical Director the preferred candidate requires a blend of clinical excellence, strategic leadership, managerial competence, and high emotional intelligence to navigate the complexities of modern healthcare. They act as the bridge between senior health management and clinical staff, aiming to improve patient outcomes while managing resources and leading diverse teams.

Participate in and facilitates education planning and delivery to undergraduate and postgraduate health professional trainees, as appropriate.

The Western Sydney Local Health District (WSLHD) Hospital in the Home (HiTH) is a well-established and mature service that delivers safe, high-quality hospital substitution care to patients in the comfort of their own homes. Operating as a fully integrated component of the acute care system, WSLHD HiTH plays a critical role in supporting patient flow and enhancing hospital capacity.

In 2024, the service over 3,500 patients who would otherwise require admission to a traditional inpatient setting. These patients receive comprehensive medical and nursing care for a wide range of acute conditions, including infections requiring intravenous antibiotics (such as cellulitis, pneumonia, and urinary tract infections), post-operative care, wound management, and other complex clinical needs that can be safely delivered in a home environment.

Since April 2022 the WSLHD inTouch Urgent Care Service has provided episodic urgent care services to persons in their place of residence. This has been achieved by finding a real-time alternative care pathway such as a rapid response referral to Hospital in the Home (HITH), outpatient clinics, mobile imaging, and incontinence (catheter care).

With a strong focus on person-centred care, early discharge, and admission avoidance, WSLHD HiTH and inTouch continues to demonstrate excellence in delivering hospital-level care beyond the walls of the hospital.

Conditions of appointment are in accordance with the Staff Specialists (State) Award

Selection Criteria

1. Possess or be eligible for registration with the Medical Board of Australia and Fellowship of the Australian College of Emergency Medicine or Royal Australian College of General Practice or Royal Australasian College of Physicians or equivalent as provided for in the Staff Specialists (State) Award/ Health Insurance Act 1997. Must have Australian Citizenship or Permanent Australian Residency. Must hold a current NSW Environment Protection Authority license (if applicable)

2. Are you a current employee within Western Sydney Local Health District?

3. Sound clinical knowledge in Emergency Medicine or General Medicine or General Practice demonstrated through successful clinical practice and experience in telehealth, community-based Medicine, preferably as part of a multidisciplinary community-based team / as part of an outreach or virtual care service

4. Evidence of additional qualifications, skills and/or experience required for specific scope of practice for the specialty

5. Demonstrated skills and attitudes appropriate for practice as a Senior Medical /Dental Practitioner including:

o Proven ability to work effectively and harmoniously with medical and other health services colleagues as part of a multi-disciplinary health care team.

o Demonstrated ability to communicate effectively with patients and colleagues.

o Excellent organisational, supervision and collaboration skills with demonstrated ability to build respectful relationships.

o Maintaining and developing knowledge, skills and professional performance to the level required for contemporary practice.

o Experience and involvement in quality assurance activities and translation of research into practice to improve patient safety and quality.

o Clinical competency in utilising digital technologies for safe and quality healthcare delivery and the adoption by other clinicians in the service.

8. Demonstrated commitment to Education, Training and Research, as applicable to the position

9. Current license to drive in NSW with the ability to drive for work purposes
