Collaborate with the Service Manager and Intake Officer to grow the program by onboarding new Support at Home clients and building your caseload.
Responsibilities
Build and manage your own caseload of Support at Home clients, ensuring timely assessments and care planning.
Coordinate and monitor services, liaising with clients, families, and service providers to meet individual needs.
Maintain accurate client records and reporting in line with organisational and regulatory requirements.
Travel within the local region to conduct home visits.
About You
Certificate IV in Ageing Support, Certificate IV in Community Services, Diploma of Community Services, or Case Management.
Valid driver's licence.
Computer literacy.
Time management and communication skills.
Knowledge and understanding of home care packages and support at home.
