Employment Type: Permanent Full-Time
Hours: 9am to 5pm
Salary: Dependent on qualifications and experience
Location: Any existing Mercy Health Home Care Offices

Position: Intake and Assessment Specialist – Home Care Team

Responsibilities
* Act as the primary contact for clients of CHSP and other programs, responding to enquiries, concerns, and changes to client needs in a professional manner.
* Conduct comprehensive, person‐centred assessments of client needs, suitability and eligibility for aged care services.
* Triage clients based on vulnerability, urgency and level of need to ensure appropriate prioritisation.
* Identify and manage risks to clients and staff during intake and review processes.
* Complete scheduled reviews for existing clients and maintain accurate, timely records and reports in line with CHSP funding and program guidelines, aged care quality standards and NDIS legislation.
* Promote Mercy's Healthy Aging services to support clients across their ageing journey.
Qualifications & Experience
* Excellent time‐management, organisational and communication skills, with the ability to adapt and problem‐solve.
* Good understanding and experience of home‐care funding programs and eligibility, including CHSP, NDIS, private and brokerage arrangements.
* Compassionate, flexible and positive approach to effective patient care and management.
* Ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
* Current NDIS check (or evidence of the process of obtaining it).
