Health System Navigator Role Summary
This position connects families, carers, and service providers to empower children and their families in navigating health systems confidently and effectively.
Main Responsibilities:
* The role ensures timely and culturally appropriate health assessments, referrals, and wrap-around care for children and young people entering out-of-home care.
* Collaborate closely with the PTGH team, VACCA, Department of Communities and Justice, carers, and regional service providers to implement quality and culturally-sensitive care plans.
Essential Skills and Qualifications:
* A relevant tertiary qualification and current registration as a Nurse, Social Worker, Aboriginal Health Practitioner or other recognized healthcare professional.
* Detailed understanding of child protection or out-of-home care systems and associated health needs.
* Proven ability to build trust, advocate for families, and coordinate care plans.
