Position Summary: The Home Care Support Worker provides person-centred care and support to clients in their own homes. The primary goal is to enhance the client's independence, wellbeing, and quality of life by assisting with daily living, domestic tasks, and community participation in line with the individual care plan.
Key Responsibilities:
1. Personal Care and Hygiene
* Assist clients with activities of daily living (ADLs) such as showering, bathing, dressing, and grooming (e.g., shaving, hair care, oral hygiene).
* Provide assistance with toileting, continence management, and changing continence aids.
* Support clients with mobility, including transfers (e.g., bed to chair), walking assistance, and the safe use of mobility aids and equipment (e.g., hoists, wheelchairs).
* Assist with the safe administration of medication reminders or support with self-administration as per the care plan.
2. Domestic Assistance and Household Tasks
* Perform light household duties, including dusting, vacuuming, sweeping, and mopping floors.
* Assist with laundry, ironing, and changing bed linen.
* Support with meal preparation, cooking, and serving meals according to dietary requirements.
* Monitor and manage general home hygiene and cleanliness to maintain a safe living environment.
3. Social Support and Community Participation
* Provide companionship, emotional support, and social engagement.
* Assist clients in accessing the community, including accompanying them to appointments (medical, social), shopping, and errands.
* Support clients to engage in hobbies, interests, and recreational activities as per their goals and preferences.
* Encourage client independence and participation in their own care and decision-making.
4. Health Monitoring and Reporting
* Observe and monitor the client's physical and emotional wellbeing, noting any changes in condition (e.g., mood, appetite, skin integrity, mobility).
* Promptly and accurately document care provided, changes in condition, incidents, or concerns, and report them to the appropriate supervisor/manager.
* Adhere to all health and safety guidelines, infection control practices, and manual handling procedures.

Qualifications and Requirements (Typical):
* A relevant qualification, such as a Certificate III in Individual Support (Aged Care, Home and Community, or Disability) or equivalent experience.
* Current First Aid and CPR Certification.
* Current Working with Vulnerable People / Police Check.
* Valid driver's license and reliable, insured vehicle for client transportation.
