About the Role
As the Koori Hospital Liaison Officer, you will support Aboriginal and Torres Strait Islander patients throughout their hospital journey, advocate for culturally appropriate care and improved health outcomes, assist with care coordination and discharge planning, provide cultural education and leadership across the organisation, strengthen partnerships with local Aboriginal organisations, and participate in community events such as NAIDOC Week and advisory committees. This largely autonomous role is supported by the Social Health Team and focuses on collaboration across services.
Responsibilities
* Support Aboriginal and Torres Strait Islander patients throughout their hospital journey
* Advocate for culturally appropriate care and improved health outcomes
* Assist with care coordination and discharge planning
* Provide cultural education and leadership across the organisation
* Strengthen partnerships with local Aboriginal organisations, including Ramahyuck District Aboriginal Corporation
* Participate in community events such as NAIDOC Week and advisory committees
About You
* Have strong connections to and understanding of the local Koori community
* Communicate effectively and build trusted relationships
* Commit to improving Aboriginal health outcomes
* Work independently and within a multidisciplinary team
Qualifications
* Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care (or equivalent)
* Demonstrated cultural knowledge and community engagement skills
* Commitment to improving Aboriginal health outcomes (required)
* Current satisfactory National Police Certificate
* Working with Children Check
