Permanent Full-Time Opportunity
Churches of Christ Queensland is seeking a Registered Nurse to join our team at Moonah Park Aged Care in Mitchelton, a 98‐bed residential Aged Care service offering respite, dementia, palliative and pastoral care supported by a compassionate team.
As our Clinical Care Coordinator you will provide advanced clinical support, leadership and coordination of care for residents with complex needs.
About the Role
As Care Coordinator, you will:
* Lead and coordinate comprehensive resident assessments within a Positive Wellbeing Model
* Ensure care plans are developed, implemented and reviewed in line with assessment outcomes and organisational timeframes
* Coordinate the accurate collection and submission of clinical and care data
* Monitor clinical documentation to ensure it accurately reflects care delivery and meets compliance requirements
* Coordinate care delivered by external service providers
* Support the review and maintenance of clinical programs, initiatives and continuous improvement activities
* Provide leadership, workforce oversight and day-to-day clinical guidance within your area of responsibility
About You
You are a confident and capable Registered Nurse with:
* Current AHPRA registration as a Registered Nurse
* Minimum three years' post-graduate experience
* Current First Aid and CPR certification
* Current Police Check, or willingness to obtain
* Australian Work Rights (Sponsorship unavailable for this role)
