Lead Care Coordinator
We are seeking a qualified professional to fill the role of Lead Care Coordinator. This is a temporary full-time position, with an initial period of up to 65 weeks for 38 hours per week.
Key Responsibilities:
* Coordinate care in the Community Mental Health Assessment Team (CMHAT)
* Manage staff under their delegation and ensure effective liaison between all health professionals
* Be responsible for environmental and work, health and safety management of the service
Requirements:
* Position Classification: Health Mgr Lvl 2, Nurse Unit Mgr Lvl 2, Occupational Therapist Lvl 4, Social Worker Lvl 4
* Must be an Australian citizen or permanent resident, or legally able to live and work in Australia
About the Role:
The successful candidate will play a vital part in delivering quality care and improving quality of life for the communities of Tweed, Richmond and Clarence Valleys. They will lead, direct and coordinate care in the Community Mental Health Assessment Team and manage staff under their delegation.
