Clinical / Comprehensive Assessor Role Overview
The Clinical/Comprehensive Assessor will conduct holistic assessments for older adults, enhancing independence and quality of life.
* Key Responsibilities:
* Conduct timely assessments that meet Department of Health timelines and quality standards.
* Liaise with families, carers, and service providers as key stakeholders in the assessment process.
* Maintain accurate documentation and reporting.
* Contribute to a culture of continuous improvement and exceptional service delivery.
* Requirements:
* Minimum 1 year of recent RAS/ACAS or SAS experience.
* SOA2 or SOA4 certification (Comprehensive Assessor transition training provided if you hold SOA2).
* Tertiary qualification in Nursing, Occupational Therapy, Physiotherapy, Allied Health, or Social Work.
* Strong knowledge of aged care legislation, CHSP, and Home Care Packages.
* Excellent communication, documentation, and interpersonal skills.
* Valid Australian driver's licence and a fully insured vehicle.
* Availability for at least 3 days per week – weekdays.
