Clinical Care Coordinator Opportunity
We're seeking a highly skilled professional to join our multidisciplinary team as a Clinical Care Coordinator. This role is part of our commitment to delivering high-quality, evidence-based clinical care to children and young people in the Central Queensland, Wide Bay, Sunshine Coast region.
The successful candidate will have expertise in working with adults and young people, as well as experience in early childhood intervention. As a key member of our consortia team, you'll contribute to the development and implementation of effective care coordination services that address service gaps for the 'missing middle'.
* Enhance accessibility and integrate primary mental health care, psychosocial support, and early intervention to address service gaps.
* Promote holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
This is a part-time opportunity with flexibility to work 4 days per week or a 9 day fortnight. You'll have the option to work from home one day per week and be based at our hub in Gympie.
To succeed in this role, you'll need:
* AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR AASW registration/eligibility as a Social Worker, OR Level 2 ACA registration/eligibility.
* Knowledge and understanding of evidence-based interventions and clinical practice in mental health and alcohol and drug use.
* Excellent communication and interpersonal skills to engage with people, advocate on their behalf, and establish effective relationships.
* Experience in electronic client/customer management systems and Microsoft Office applications.
* Eligibility for a Blue Card and NDIS worker screening clearance for paid employment.
