The MCCN provides care, information, and support for people with cancer across the continuity of care pathways and multidisciplinary teams within the cancer care community. The MCCN will provide collaborative clinical and supportive care for people and their families, with leadership that facilitates ongoing development of clinical practice that reflects the McGrath Model of Care. The role includes clinical advice, leadership in research-based practice, education, research and professional leadership, and care coordination for patients with complex needs.
Responsibilities
Provide expert clinical advice to patients/clients, carers, and other health care professionals with complex needs within Cancer Services.
Lead and facilitate development of clinical practice that reflects the McGrath Model of Care.
Develop, implement, and evaluate care management plans for patients diagnosed with specific cancers, focusing on care coordination for those with complex needs.
Develop education and training programmes; participate in collaborative research with other health professionals in relation to Cancer Care Services.
Provide expert consultation and advice to health care workers for patients with special or complex needs through the continuum of care until discharge, with follow-up as needed.
Establish and maintain communication with Medical Practitioners, Nursing and Allied Health Professionals; participate in multidisciplinary team meetings; liaise with Division of General Practitioners, Community GPs and community health services.
Coordinate scheduling of treatments, triage patient needs, and make timely referrals to allied health and support services.
Assist with discharge planning and ensure information is tailored to meet patient and carer needs.
Participate in the multidisciplinary team and contribute to treatment planning; ensure data required by the Foundation is added to the McGrath Foundation database; participate in Foundation education and information sharing events.
Provide information and education to patients and families; support continuity of care from diagnosis through treatment to discharge or terminal care.
Qualifications and Requirements
Applicable vaccination requirements for a Category A role.
Working With Children Check (WWCC).
Current AHPRA Registration.
Minimum five years full-time equivalent post-registration experience, with at least three years recent experience in Oncology, Palliative Care or Cancer Stream nursing; postgraduate qualifications relevant to the field of practice.
Valid NSW Driver's Licence.
This is a temporarily funded position until 27 June 2027 with the possibility of extension subject to funding.
