About this Role
This position involves delivering an effective and efficient social care service that addresses the psychosocial aspects of patient care.
Key responsibilities include:
* Providing intervention, support, education, and resources to individuals in their bio-psycho-social situation.
* Completing comprehensive assessments and developing intervention plans as required.
* Organising and facilitating family meetings, and coordinating complex case management plans.
The successful candidate will have a degree in Social Work or an equivalent qualification, demonstrated knowledge and understanding of the Social Work role in healthcare, and commitment to working collaboratively within a team approach.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders. They will also be able to maintain confidentiality and handle sensitive information discreetly.
Additional responsibilities may include collaborating with other healthcare professionals, participating in quality improvement initiatives, and contributing to the development of policies and procedures.
