The Role

This role provides exceptional leadership to enhance frontline services for the people we support and stakeholders, whilst ensuring we have the engaged people, quality services and enduring partnerships.
Bright Futures is the service name for Outcare's work with young people in Out of Home Care. The Bright Futures Case Manager provides case management and coordinated support to young people participating in the Bright Futures program.
The role works with young people experiencing complex social challenges, including justice involvement, trauma, mental health concerns and housing instability. Using therapeutic, trauma-informed and strengths-based approaches, the Case Manager supports young people in developing life skills, building resilience and progressing toward positive educational, employment and social outcomes.
Working within the residential program environment, the Case Manager coordinates the implementation of individual support plans and works closely with Bright Futures Residential Workers to ensure consistent, safe and supportive care for young people. The role collaborates with the Bright Futures Team Leader and external stakeholders to ensure services are delivered in line with program frameworks and organisational requirements.
What you'll bring
* Relevant qualification in Youth Work, Community Services, Social Work or a related discipline (or equivalent experience).
* Experience supporting young people with complex needs within youth services, residential care, justice or community service environments.
* Demonstrated ability to develop and implement person‐centred support or case management plans.
* Understanding of trauma‐informed, strengths‐based and culturally responsive practice.
* Ability to build positive relationships and engage effectively with young people.
* Strong written and verbal communication skills, including maintaining accurate case notes and records.
* Ability to manage a caseload and coordinate services across multiple stakeholders.
* Experience coordinating or supervising support staff within youth services, residential care or community service environments.
* Ability to provide guidance and direction to support workers delivering daily care and support.
* Ability to identify and elevate risks relating to safety and wellbeing
