About the role
This role is responsible for coordinating and overseeing the provision of physiotherapy services for our community home care customers. You will be involved in carrying out assessments for customers with complex care needs and creating individual care plans. This position gives you the flexibility to plan your own schedule and work autonomously on the road, where no two days are ever the same. This role does require you to be travelling to clients out in the community, who range geographically from Capel further south, to Cowaramup/Dunsborough. Our customer base is continuously growing, so ideally, we are looking for someone who can commit to 4-5 days per week, but we are flexible for the right candidate.
Key responsibilities
* Plan and implement physiotherapy services for community clients using an individual-centred approach
* Complete assessments for new community clients and conduct regular reviews to identify client needs
* Plan optimum physiotherapy programs and provide appropriate treatments and instructions
* Work as part of a multi-disciplinary team that is committed to the overall physical and emotional health of our clients
* Maintaining care plans and providing advice to our front‐line care staff

Experience and qualifications required
* Tertiary Physiotherapy Qualifications
* Current registration with AHPRA as a Physiotherapist
* Current and valid C Class Drivers Licence
* Access to reliable own vehicle
* Proof of WA Vehicle Registration and Comprehensive Car Insurance
* Willingness to obtain a National Police Clearance
* Previous experience within aged care (highly desirable)
* Sound understanding of people living with dementia and/or people living with complex care needs.
