Community Health Nurse Position
We are seeking a dedicated and compassionate Registered Nurse to join our multidisciplinary team in providing high-quality care to older adults in the community.
* About the Role:
This is an exciting opportunity for a motivated Registered Nurse to work in a dynamic community setting, supporting patients to regain their independence and quality of life.
* Key Responsibilities:
1. Deliver efficient and responsive care to patients recently discharged from hospital, promoting their physical and emotional well-being.
2. Conduct thorough assessments, develop and implement individualized care plans, and coordinate ongoing care arrangements with other healthcare professionals.
3. Collaborate closely with a multidisciplinary team, including RNs, ENs, Physiotherapists, Occupational Therapists, and Domestic Support staff, to ensure seamless care delivery.
4. Provide leadership and guidance to team members across both Taree and Forster locations, promoting a culture of excellence and continuous improvement.
* Requirements:
1. A registered nurse with a strong commitment to person-centered care and aged health services.
2. Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
3. A passion for working collaboratively as part of a multidisciplinary team, with the ability to lead and support fellow nurses and allied health staff.
