Home Healthcare Professional
We are seeking an experienced Community Registered Nurse to provide in-home clinical nursing services, conducting holistic assessments, educating clients and carers on self-management, and coordinating services that support older Australians to live well and independently at home.
This role involves delivering wound care, medication management, catheter care, and chronic disease support. It also entails developing care plans with a focus on reablement, reviewing outcomes, and educating clients and carers on health literacy to promote independence.
* Provide clinical care autonomously in home settings, utilising strong clinical judgement and problem-solving skills.
* Work collaboratively with multidisciplinary teams to ensure comprehensive client care.
About You
* AHPRA-registered RN with a minimum of 1 year of experience in community, primary health, aged care, or similar sectors.
* Demonstrate confidence in delivering clinical care in autonomous settings.
* Exhibit strong organisational skills, effective communication, and a solutions-focused mindset.
