Clinical Care Partner
The Clinical Care Partner is an innovative role within the Home Care Package Program that involves providing support and assistance to consumers as a partner in the coordination and management of care and services.
This position requires an enthusiastic professional with a real passion for improving the lives of our consumers and supporting a team of health professionals to deliver quality consumer care and support.
The successful candidate will be responsible for coordinating all elements of the consumers' Home Care Packages, including service intake, coordination of home care and support, consumer agreements, support planning, referral, and care-management.
* Support and assist consumers as a partner in the coordination and management of care and services within the Home Care Package Program.
* Coordinate all elements of the consumers' Home Care Packages, including service intake, coordination of home care and support, consumer agreements, support planning, referral, and care-management.
* Contribute to a multidisciplinary team approach and manage the HCP budget allocation in partnership with the consumer.
* Acknowledge and document all consumer records accurately and timely.
Requirements:
* Registered or Enrolled Nurse.
* Experience working in an aged, health or community setting.
* Current motor vehicle driver's licence.
* Two professional referee checks worked with candidate recently.
* National Police Certificate obtained within the last three months.
* Working with Vulnerable People card.
