Job Title: Disability Support Specialist


Overview


Welcome to our organization, one of the largest providers to the NDIS, supporting over 10,000 participants every year with the care they need, when and where it is needed.


We have a multi-disciplinary team approach, building a quality support service around the needs and goals of our participants and their family. Our team helps improve access, development, independence, and overall health and wellbeing in individuals with disabilities.


Position


We're looking for an experienced Disability Support Practitioner to join our team, supporting NDIS participants to achieve their goals and live more independently.


As part of this role, you'll be providing therapeutic supports to participants living in various regions, working collaboratively to develop and implement positive behaviour support plans that make a meaningful difference.



* Supporting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning.

* Evaluating, developing and delivering evidence-based support plans within a complex support space.

* Developing meaningful goals and practical strategies as per the positive behaviour support framework, with a commitment to eliminate restrictive practices, in the home and community.

* Ensuring individual plans reflect participants' goals, passions and strengths.

* Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc.

* Working collaboratively within a multidisciplinary team, including mentoring less experienced Disability Support Practitioners.

Qualifications


Current suitability at Core, Proficient or Advanced level as a Behaviour Support Practitioner.


Considerable experience providing support to people with behaviours of concern (professionally and personally) is essential.


Completed a degree-level qualification as an allied health professional is desired.


Report writing experience.


Excellent time management and organisational skills to manage a varied caseload.


Proactive attitude, motivated by improvement and the opportunity to test and learn from new ideas.


Genuine passion for working in the disability sector.
