We are seeking a dedicated and compassionate individual to join our team as a Youth Support Worker. As an integral part of our organization, you will play a vital role in providing safe and nurturing environments for young men and women who are homeless or at risk.
Key Responsibilities:
* Provide professional support to residents under the Journey to Independence Model, promoting their physical and emotional well-being.
* Maintain accurate records and ensure effective handovers to ensure continuity of care.
* Support residents in following case management plans and attending appointments, empowering them to take control of their lives.
* Offer informal support to all program clients and identify risks and service gaps, contributing to the development of targeted interventions.
Requirements:
* A tertiary qualification in community services or a related field is essential for this role.
* Demonstrated experience in a social service environment is required, with a strong understanding of best practices and evidence-based approaches.
* A current VIC drivers licence and Working with Children's Check are mandatory, ensuring your safety and the safety of those you serve.
