Your Opportunity

As a Case Manager in the THRIVE service based in Geraldton, you will work face-to-face with people who are navigating complex issues that threaten their housing stability. This is an on-site position where your presence and support make a real difference every day. THRIVE supports tenants of Public Housing and Aboriginal Housing who are experiencing challenges in sustaining their tenancy by providing case management, practical assistance, and connections to community resources.

You will partner with tenants, listening to their stories, understanding their needs, and helping them build the skills and confidence to sustain a safe and secure home. Your role goes beyond paperwork, it's about creating trust, fostering resilience, and connecting people to the right community resources. Whether it's developing a tailored case plan, conducting home visits, or collaborating with local service providers, you will be the link that turns uncertainty into stability.

Your Key Responsibilities

* Accept and respond to referrals from the Department of Communities, regional service providers, and self-referrals, conducting initial assessments and inductions.
* Develop and maintain individualised, strength-based case plans using SMART goals, with regular reviews to track progress.
* Provide face-to-face meetings and home visits to support clients in maintaining their tenancy, including inspections where required.
* Facilitate referrals to internal and external support services to address complex client needs.
* Undertake risk assessments and ensure compliance with Mission Australia's case management protocols and quality standards.
* Maintain accurate client records and complete data entry in relevant systems promptly and in line with organisational requirements.
* Collaborate effectively with the Department of Communities (Housing) and other stakeholders to achieve sustainable housing outcomes.

Requirements for Success

* Minimum qualification of a relevant degree, associate diploma, or certificate with experience, or equivalent expertise gained through previous roles or study.
* Demonstrated experience working with clients facing complex challenges, including housing instability and distress.
* Strong communication skills, both written and verbal, with proficiency in administrative tasks and computer systems.
* Excellent understanding of Aboriginal and Torres Strait Islander peoples, cultures, and protocols.
* Ability to maintain professional boundaries while building client capacity and resilience.
* Proven capability to work with diverse communities and manage cases with sensitivity and cultural awareness.
* Strong local knowledge and established connections within the Geraldton and surrounding community.
* First Aid and CPR Certificate.
* Full driver licence with no restrictions.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
