Primary Care Coordinator | Aged Care Dunborough
$57.43 – $59.43 per hour + super + salary packaging
About the Role
The Primary Care Coordinator is responsible for developing and maintaining partnerships between Capecare residential aged care homes (RACH), general practitioners (GPs), allied health professionals, pharmacists, and other relevant community health care providers. Based in the Dunsborough facility, this role facilitates optimal communication and integration of primary care services for Capecare residents.
Key Responsibilities
* Develop strong collaborative relationships with service providers such as GPs, allied health, and pharmacists.
* Collaborate with GPs to enable efficient, timely, and responsive primary care for residents.
* Facilitate communication among GPs, Capecare staff, residents and family/carers.
* Coordinate communication with other areas of the healthcare sector and Capecare.
* Support knowledge and skill development among clinical and care staff.
* Work with the Quality & Clinical Governance Manager to develop clinical protocols with GPs, guide decision making, triage, assessment and escalation.
* Maintain oversight of GPs providing treatment to residents.
* Develop and manage an Implementation Working Group in line with GPAIC guidelines, including Capecare senior clinical leaders, GPs and WAPHA activity lead.
About You
You are an experienced Registered Nurse who is a strong relationship builder, a confident communicator and a natural networker. You understand the importance of coordinated care and are passionate about improving communication between providers to enhance healthcare quality, accessibility and continuity for residents.
Qualifications & Experience
* Bachelor of Nursing – AHPRA registration
* Postgraduate qualifications in dementia care, palliative care, primary care or mental health nursing (desirable)
* Minimum of 5 years' experience in aged care
* Experience in dementia and palliative care
* Demonstrated knowledge of the Age Care Act and Standards
