Rehabilitation and Geriatric Care Nurse Opportunity
We are seeking a dedicated Rehabilitation and Geriatric Care Nurse to join our multidisciplinary team in the Mary Coram Unit. This is an exciting opportunity to work with patients who require rehabilitation and geriatric evaluation and management.
Job Summary:
The successful candidate will be responsible for providing high-quality, patient-centred care to patients with complex medical conditions. The nurse will work collaboratively with the multidisciplinary team to develop individualised care plans that meet the unique needs of each patient.
Key Responsibilities:
* Provide safe and effective care to patients with stroke, neurological diagnoses, orthopaedic fractures or replacements, deconditioning post-acute illness, surgery or trauma, and complex medical conditions related to ageing;
* Collaborate with the multidisciplinary team to develop and implement individualised care plans;
* Contribute to the development of comprehensive care plans that address the physical, emotional, and psychological needs of patients;
* Work effectively in a fast-paced environment to manage multiple priorities and deadlines;
* Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery;
* Participate in quality improvement initiatives to enhance patient outcomes and staff satisfaction;
* Develop and maintain skills in relevant areas such as palliative care, medication management, and wound care;
* Support and supervise junior nurses and healthcare assistants to promote professional growth and development;
* Engage in continuous learning and professional development to stay up-to-date with best practices and research in rehabilitation and geriatric care.
