**Purpose of the position**
YourBridge Cares is committed to following each participant's Individual Care Plan (ICP) and providing the highest standard of care with the aim of building each participant's capacity and maximising their quality of life. The primary role of a Disability Support Worker is to help individuals with physical and / or psychosocial disabilities perform basic hygiene tasks, such as bathing, brushing teeth, grooming, dressing in addition to daily living tasks such as cooking, cleaning, and doing laundry in their private home or SIL accommodation. The role also involves accompanying and supporting the participant with engaging in social and wellbeing activities in the community.

**Responsibilities & duties**
- Accompanying and supporting the participant on various outings to increase social and community access and participation.
- Providing companionship to clients who are socially isolated.
- Supporting clients with home administration (e.g., paying bills, making appointments, assistance with the use of technologies etc.)
- Introducing, encouraging, and participating in activities that support the participant's goals, strengthen skills, and provide enjoyment (e.g., gardening, cooking, painting, reading, gaming, listening to music, art and craft, dancing, board games, PS Games, Nintendo games etc.)
- Consulting with families, carers, and allied health teams to optimally support the client; you may support the implementation of therapeutic interventions where appropriate.
- Assist with the assessment of clients and the development of individual recovery plans to aid clients in reaching their recovery goals.

**Work standard**:
- All clients receive a high level of care and support as per their Individual Care Plan.
- All forms of support are provided to clients in a caring and supportive way.
- Documented evidence demonstrates that service procedures are followed.
- Daily documentation completed (i.e., support logs, case notes).
- All policies and procedures are followed.
- The Staff Code of Conduct is adhered to.
- WHS instruction and procedures are followed exactly.
- Regular review of all mandatory policies and procedures on staff Intranet computer systems to ensure effective service delivery and compliance with legislation.

- Display a commitment to encouraging participant's in achieving their goals listed in their NDIS plan

**Work experience & skills**
**Essential experience**
- Experience as a disability support worker
- Comfortable working autonomously and independently
- Excellent interpersonal and communication skills
- Able to adapt your approach to suit the individual you are working with.
- Demonstrated ability to develop individual support plans with participants.
- Ability to follow a Positive Behaviour Support Plans and implement strategies to meet the individual's needs.
- Understanding of Restrictive Practices
- Understanding of Incident Reporting and Management
- **Qualifications and Experience**_
- Certificate IV in Disability or Mental Health and related field experience working with individuals with Autism Spectrum, Intellectual Disability and severe Mental Illness.
- Experience working well within a Care Team supporting a participant ongoing.
