About the role
Calvary Adelaide Hospital is the largest private hospital in South Australia and we are seeking an experienced Social worker to join our dynamic multidisciplinary Allied Health team.
As a member of the multidisciplinary Allied Health team, the Social Worker will provide high quality clinical social work services across a range of clinical areas in accordance with the prescribed professional and ethical standards. The Social Worker will report to the Allied Health Manager and predominantly work with patients in acute and rehabilitation settings at Calvary Adelaide Hospital.
Key Responsibilities
Practice to highest professional standards as guided by the Australian Association of Social Workers
Promote high quality patient outcomes for patents and their families, including liaison and negotiation in complex case management, through involvement in the interdisciplinary team, networking and education
Ensure the patient voice is included in all aspects of holistic patient care including safety, information and responding appropriately to any acts of unprofessional conduct
Promote open and honest communication between all health care providers, the patient and their family
Establishment of sustainable resources for consistent navigation through complex systems such as the NDIS and SACAT
To actively participate in discharge planning with patients, families and the health care team
Participate in staff educational and training opportunities and provide in-service training to other staff as appropriate
What you bring
A degree or other qualification that entitles registration as a social work discipline, enabling eligibility for membership to the AASW
Minimum 2 years of experience in an acute health care setting
My Aged Care ACAT Assessor qualification or willingness to become an ACAT affiliate assessor
Ability to work under minimum supervision as part of a multidisciplinary team
Experience in service provision for people with diverse backgrounds
Extensive knowledge of Australian Association of Social Workers Code of Ethics - AASW and Practice Standards - AASW
Ability to work in partnership with patients and their relatives to meet their spiritual and cultural needs
Excellent time management skills
Strong verbal and written communication skills
Capacity and skills to provided a broad range of social work services for acute and rehabilitation settings
Comprehensive knowledge and understanding of a broad range of psychosocial issues including family conflict, trauma, acquired disability, domestic violence, elder abuse, child protection, mental illness, cognitive decline and issues associated with terminal illness, and bereavement and grief situations
A desire to join a team committed to the care of others in a mission based organisation
