About The Organisation

We are currently seeking experienced Support Workers to join our growing team across the Perth metropolitan region. We have a range of shifts available, including morning, afternoon, active night and sleepover shifts. Please indicate your availability and preferred shift type in your application.

Key Responsibilities

* Support participants to work towards achieving their individual goals in line with their support plans
* Assist with daily living activities, including personal care, cooking, cleaning, shopping and community access
* Support participants to build confidence, independence and life skills
* Assist with medication administration in line with organisational policies and training
* Implement positive behaviour support strategies
* Encourage participation in age-appropriate activities, employment and social engagement
* Maintain a safe, respectful and supportive environment at all times
* Monitor participant wellbeing and report any concerns to the Manager
* Accurately complete required documentation, progress notes and communication records
* Participate in individual planning processes and monitor outcomes
* Liaise with families, carers, case managers, allied health professionals and community agencies
* Work in line with NDIS standards, WHS guidelines, privacy and confidentiality requirements
* Collaborate with team members to ensure consistent and high-quality service delivery

Skills & Experience

* Qualification preferably in community/disability/individual support/aged care/mental health and/or relevant experience
* Experience working with clients with challenging behaviours
* Well-developed verbal and written communication skills
* Current Driver Licence and reliable vehicle
* 100-point ID check
* Valid visa/working rights in Australia

* Manual Handling Certificate
* Working With Children Check
* NDIS Worker Screening Check
* NDIS Orientation Module Certificate
* Hand Hygiene Certificate
* NHHI Infection Control Certificate
