Job Title: Midwife - Continuity of Care Specialist
This role presents an exceptional opportunity for a skilled Midwife to provide continuity of care to women and their families in our caseload midwifery led model. As a key member of our multidisciplinary team, you will work collaboratively with local obstetric teams to deliver high-quality patient-centered care throughout the continuum of pregnancy, labour, birth, and the postnatal period.
Key Responsibilities:
* Provide antenatal and postnatal support and backup to women within the community or hospital setting
* Collaborate with a multidisciplinary team to ensure seamless care transitions and outcomes
* Develop and maintain strong relationships with women and their families to promote empowerment and informed decision-making
* Contribute to the development and implementation of evidence-based clinical guidelines and protocols
* Participate in quality improvement initiatives and contribute to a culture of safety and excellence
Requirements:
* Minimum 3 years demonstrated post-registration experience working in midwifery-led, women-centred models of care
* Advanced skills across the continuum of care, including antenatal, intrapartum, and postnatal care
* Effective communication and interpretation skills, with the ability to facilitate information sharing between colleagues, women, and their families
* Ability to work as a member of a multidisciplinary healthcare team, promoting collaboration and mutual respect
* Commitment to ongoing professional development and staying up-to-date with best practice and research in midwifery
