**Superior Home Health Services** are seeking a qualified, reliable and passionate Home and Community **Enrolled Nurse Care Coordinator** to join our team at the **Gympie - Regional Office.**
**Responsibilities will include**:
- Working within a person-centered, enabling environment that respects the rights and needs of each client;
- Manage case management for a variety of aged and disability clients;
- Manage Home Care Package funding administrative tasks;
- Assisting clients with personal hygiene, dressing, and domestic duties;
- Assisting clients with daily errands and transport where required;
- Providing support to access social and recreational activities;
- Assisting clients with medications;
- Observe, care and report any changes in the health and wellbeing of the client;
- Work under our RN Clinical Care Advisor to undertake skilled nursing tasks,
- Responding to emergency situations
**Essential Criteria**:
- Minimum Diploma of Nursing
- Excellent verbal and written communication skills
- Willingness to learn, develop and take on new challenges
- Current National Police Check
- Current First Aid & CPR certificates
- Current unrestricted driver's license
- Medication Assistance
- A reliable, well-maintained vehicle
- Comprehensive car insurance
- NDIS Worker Screening including Blue Card (or equivalent)
- Community experience, 1 year minimum
Your professional, friendly and welcoming personality, along with your great communication and positive attitude, will serve you well as you will be first point of contact, for our clients and their families and our care delivery team.
In this role you will be responsible for liaising between many stakeholders including clients, or their families or representatives, the clinical team, care delivery team, scheduling team, corporate services team, our referral partners, and industry bodies. Ideally you will have at least 12 months experience in a similar role.
Pay: $60,000.00 - $75,000.00 per year

**Experience**:
- Community: 1 year (preferred)

- Qualification relating to Nursing, Aged Care, or Disability (required)
