**We are seeking a Home Care Case Manager to join a growing business**

**Who Are We?**

Community Home Care Service is a small Home Care Package and NDIS provider made up of a team of dedicated and skilled professionals who take pride in their work. We are located in the heart of Western Sydney with home care clients all over Sydney.

**The Opportunity**

We have a permanent full-time position - Monday to Friday, 9 to 5 (open to negotiation) - available for an experienced Case Manager to join our growing team. You will have the opportunity to support our established client base. You will be provided with a computer and mobile phone as well as a supportive admin team to help you with administrative tasks.

We pride ourselves with being flexible and understanding of everyone's unique circumstances and work together to meet your needs. We are a close knit team that feel comfortable asking each other for assistance. You will have the opportunity to shadow an experienced case manager initially, followed by constant opportunities to debrief and have supervision as required.

We frequently have lunch together, celebrate birthdays and and important milestones in the business. We do team activities to encourage team building and inclusiveness.

**As a Support Coordinator you will be**
- Developing, implementing and managing Home Care Packages
- Building and maintaining strong working relationships with a number of stake holders including the client's family and allied health providers
- Partnering with consumers in the development and implementation of their consumed driven care plans and continue to provide them with ongoing support
- Ensuring that each client is aware of their budget and the package is managed within its budget
- Maintain accurate and up-to-date documentation in line with company's policies and procedures

**What we need from you**
- At least 1 year experience in this or similar field (preferred)
- A high level of competency in the English (required)
- Computer skills with a working knowledge of Microsoft Office Suite and the ability to learn a new database
- Excellent time management and organization skills
- Eagerness to learn and stay up-to-date with changes in regulations
- Excellent communication and interpersonal skills
- Enthusiasm to work as part of a dynamic team

Pay: $65,823.55 - $75,000.00 per year
