Summary:
We are seeking a highly organised and hardworking Case Manager to join our team. The Case Manager will be responsible for managing and coordinating the care of children and young people, ensuring that they receive the appropriate services and resources to meet their needs.
Responsibilities:
* Undertake assessments, provide case management, crisis intervention, outreach, advocacy and supported referrals to young people.
* Provide assessment, support and case management to Youth Workers
* Create and update fortnightly rosters and communicate with Youth Workers
* Review and manage incident reports
* Maximise flexibility in treatment and support services to that, where possible and appropriate, young people can maintain their current environment with minimal disruption to themselves and others.
* Actively participate in developing and implementing innovative models of service delivery in response to young peoples needs.
* Advocate on behalf of young persons to ensure appropriate service delivery to meet their identified needs.
* Liaise and consult with relevant government bodies and community agencies and develop inter-service networks and linkages to ensure appropriate and coordinated ongoing case coordination and referral processes.
* Administrative duties including follow-up with each houses help desk requests and liaise with the Administration team to ensure requirements are actioned
Qualifications and requirements:
* Minimum Certificate IV in Youth Work or relevant field and/or experience on the ground as a Youth Worker and/or Case Manager
* Current Working with Children Check (Blue Card)
* Current Open Drivers License
* Current First Aid & CPR Certificate
* Knowledge of community resources and services
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong organisational and time management skills
* Ability to maintain confidentiality and handle sensitive information
* Proficiency in Microsoft Office
