About the facility
Community Nurses provide 24hour in home nursing care in the Geelong region working in close partnership with our hospitals, allied health professionals, general practitioners and community organisations to support clients in their home environment.

A unique opportunity based within one of our three integrated nursing teams nurses are supported clinically and professionally with:
- An extensive structured orientation and on‑boarding period
- An assigned experienced preceptor/mentor.
- Community Nurse educator
- Ongoing education and professional development
- Supernumerary days within the broader community health programs
- Professional Development leave to develop the foundational knowledge and skills specific to home based nursing
- Regular debrief and feedback sessions
Be responsible for client assessment and care planning for a diverse range of clients whilst delivering clinical care including:
- Acute and chronic wound assessment and management
- Diabetes education and monitoring
- Continence management
- Hospital in the Home service
- Support and care of the terminally ill (in home palliative care).
There are three Community Nursing Specialist Year roles available:
- North (Based at the McKellar Centre) – 68hrs a fortnight
- East (Based at Newcomb) – 64hrs a fortnight
- South (Based at Belmont) – 68hrs a fortnight
In this role you will be rostered to a 4 week rotating roster which includes weekend shifts and afternoons:
- Day shift: 0730 – 1600hrs
- Afternoon shift: 1300 – 2130hrs
All shifts commence and finish at your assigned Community Health Centre. Vehicles, phones and personal duress alarms are provided for use during your shift.
3+ years of acute nursing skills and experience is desirable. This position requires a motivated and enthusiastic individual who is committed to ongoing professional development. The successful applicant will have sound critical thinking skills and behaviours that align with the Barwon Health values.
