About This Opportunity
We are seeking a skilled professional to coordinate services for the Commonwealth Home Support Program (CHSP) clients. Your role will involve providing excellent customer service, ensuring all inquiries are addressed promptly and professionally.
You will be responsible for coordinating the intake of clients and scheduling of services, as well as leading a team of Home Care Assistants to deliver quality Home Care services. This will include mentoring and training staff, providing learning opportunities, and supporting staff to deliver high-quality care that promotes client choice and independence.
To succeed in this role, you will need to be passionate about delivering high-quality services, have excellent coordination skills, and ideally experience within the CHSP program. You will also need to be able to communicate effectively and foster a great team environment.
