Our organization is seeking an experienced and compassionate individual to fill the role of Aged Care Comprehensive Assessor. This is a critical position that requires a deep understanding of older adults' needs and the ability to connect them with services that promote independent living.
Key Responsibilities:
* Conduct thorough assessments to identify the needs of older adults, ensuring their safety and well-being are prioritized at all times.
* Facilitate referrals to essential services that support independent living, working closely with families and carers to ensure seamless care coordination.
* Promote functionality by actively engaging with clients and their loved ones to maximize retention and capability in daily living activities.
* Ensure compliance with Department of Health contractual requirements, maintaining accurate records and adhering to regulatory standards.
Requirements:
* Minimum 1 year experience in aged care, including client assessments and/or service delivery.
* Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
* Current unrestricted General Registration with AHPRA or relevant professional association.
* Proficiency in Word, Excel, Outlook, and client management systems.
* Current Australian driver's licence, comprehensively insured vehicle, and the ability to travel within your service area.
