Clinical Care Coordinator & Nurse Mentor
Role Overview
Join the Tweed Clinical Care team in a rewarding fast‐paced role as a Care Coordinator and Nurse Mentor. This full‐time position reports to the General Manager and works closely with the Clinical Care Manager while supervising and supporting the care team. The key purpose is to ensure all residents receive the highest quality of clinical care.
Key Responsibilities
* Provide strategic and operational leadership to the clinical care team to ensure high‐quality, resident‐centred care.
* Oversee the development, implementation, and regular review of individualised care plans aligned with best practice.
* Monitor clinical activities, including mealtimes and key daily events, to maintain service excellence.
* Mentor and support RNs, ENs and PCWs to strengthen clinical capability, teamwork and professional growth.
* Facilitate effective communication processes such as shift handovers, clinical huddles and team meetings.
* Provide oversight of clinical decision‐making, ensuring compliance with evidence‐based practice and regulatory standards.
* Manage complex resident care needs, including specialist referrals, case reviews and family consultations.
* Lead onboarding, training and ongoing development of care staff, identifying skill gaps and supporting performance management.
* Monitor resident health changes, elevate concerns promptly and implement strategies to minimise risks such as falls and infections.
* Participate in clinical audits, maintain accurate documentation and support quality improvement initiatives aligned with Aged Care Quality Standards.
* Coordinate daily workflow, staffing and resource allocation while maintaining communication with senior leadership on team performance and resident outcomes.
Qualifications
* Minimum 2 – 3 years experience as a Clinical Care Coordinator or similar in an aged care setting.
* Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
* Post‐graduate qualifications in aged care are advantageous but not essential.
* Experience and extensive knowledge of aged care clinical assessments, care plans, AN‐ACC and accreditation compliance and documentation.
* Engaging style that can influence and develop clinical leadership skills across the clinical care team.
* High level of computing skills to maintain documentation and reporting in accordance with organisational and legislative requirements.
* Excellent communication and interpersonal skills with a passion for leading clinical outcomes.
* Ability to manage multiple stakeholders and handle emergency and stressful situations calmly.
